Nutrition and weight status are listed as topics of interest on the document Healthy People 2020 (Office of Disease Prevention and Health Promotion [ODPHP], 2020).  There is much scientific data supporting the intake of good nutrition and the maintenance of a healthy weight, and in addition to learning about food and drink choices that increase health, we must examine cultural considerations that include physical and social determinants surrounding an individuals nutrition and weight status.     
            Physical determinants of food choices are related to availability of foods and an individuals ability to purchase what they need in order to maintain a healthy body and mind.  Retail markets that sell healthy food may be less accessible in rural communities and for those with a lower-income bracket (ODPHP, 2020).  Options around that scenario include personal or community gardening, getting fresh eggs from a community member who raises chicks, and buying fresh produce from roadside stands or from a local farmers market.
            In addition to physical determinants of food choices, there are social factors that influence choices of foods and degree of healthiness of the foods.  These include:  attitudes about certain foods, stigma surrounding that ones food choices that may cause judgment from others, price of foods, knowledge and skills about what is healthy and how to prepare a meal, and cultural norms that exist in the individuals home, social circle and community.  If an individual is participating in a food assistance program, that will also determine their food choices as they get foods that are covered by the program.
            Healthy eating, healthy lifestyle, regular physical activity and balancing intake with calories burned, are vital keys to maintaining a healthy weight status.  The American Academy of Pediatrics (American Academy of Pediatrics [AAP], 2018) supports healthy family-based lifestyle modifications to help facilitate optimal nutrition and weight status.  These include preparing meals at home as a family, parents/caregivers modeling healthy choices and behavior, providing accessibility of healthy foods and drinks, participating in physical activity on a regular basis, and avoiding too much time spent on sedentary activities.
            APRNs play a role in affecting culturally-based patient care by using evidence-based practices in the following ways:  reinforcing healthy eating within the patients food preferences and belief system, physical activity that supports a healthy life withing the patients cultural preferences, discouraging skipping meals and diet pills, reinforcing the value of having meals as a family where possible, reinforcing positive discussions related to body weight, and monitoring weight loss (AAP, 2018).  APRNs address cultural and spiritual considerations when they provide focused attention to their patient preferences and ask what they can do to support their patients healthy nutrition and weight status.
