The patient has died in the hospital (in today’s craziness with COVID we really do not have to imagine)

And in your mind come up with all possible (and impossible) situations/events which might lead to this death.

Use the Cause and Effect Diagram (Fishbone) to find underlying cause of increase number of hospital deaths. There’s no paper due, just fill in the diagram.
